Well I have my own theory to this. Perhaps the source of lycanthropy (enzymes, magic, whatever it is) eventually wears down and produces overall weaker werewolves if it's passed only through natural reproduction. Peter could be as small and weak as he is simply because whatever it is that makes him a werewolf has been passed through birth for several generations and hasn't had a chance to "attack new blood" so to speak. Since it was pent up for so long it just simply exploded when exposed to Sarah's bloodstream and made her huge.
This would suggest that, while dangerous to do so, it would be vital to spread lycanthropy by infecting humans now and then rather than solely through standard birth among already existing werewolves.
